Procedure:

  1. Review the information on the above-mentioned pages
  2. Fill out the form completely and correctly with the required information and sign
  3. Bring it to our office (ITZ 166, Innstr. 43) or drop it in our mailbox in front of the foyer (ITZ 164) for review, verification, and signature/stamp by the professor (if necessary, we will contact you to schedule an appointment, if further clarification is needed)
  4. We will contact you by email (please ensure your email address on the application is legible) as soon as you can pick up your documents.
